Seizures, Takotsubo syndrome, and sudden unexpected death in epilepsy: A still puzzling triangle

چکیده
I immensely enjoyed the contribution by Finsterer and Bersano [1], published in the September 2015 issue of the Journal, about seizures, Takotsubo syndrome (TTS), and sudden unexpected death in epilepsy (SUDEP). The authors carried out a review of the heretofore reported cases of TTS triggered by seizures, with particular intension of evaluating fatal outcome. Among the reported 74 patients (86% female) with seizure-triggered TTS, only 2 (3%) died. Seizures experienced by these patients included generalized tonic clonic seizures, generalized status epilepticus, and complex partial seizures, and the underlying plausible cause of the seizures was noted. The authors concluded that ‘‘fatalities are rare in patients experiencing seizure-triggered TTS’’, and that ‘‘seizure-triggered TTS does not seem to play a major role in the pathogenesis of SUDEP’’ [1]. I shall appreciate the response of the authors on the following: (1) Since seizures, after subarachnoid hemorrhage, are the most common central nervous system triggering TTS, is it possible that we are missing milder, atypical, subclinical forms of TTS? [2,3]. (2) It is conceivable that such milder forms of seizure-triggered TTS, that do not result in hospitalization, after control of the seizures, leave behind some electrocardiographic hallmarks, which could be detected by ‘‘smartphone’’-based technology monitoring, carried out by the patients themselves, or their relatives and other caregivers [3,4]. (3) How do we know that all or many of the victims of unexplainable SUDEP, had not suffered a seizure-triggering TTS, that led to an immediate or early demise, due to a ventricular arrhythmia (monomorphic or polymorphic ventricular tachycardria, degenerating in ventricular

Takotsubo syndrome (TKS): A possible mechanism of sudden unexplained death in epilepsy (SUDEP)
We report a case of Takotsubo syndrome after epilepsy, and review the literature. We identified 59 cases of Takotsubo syndrome after focal or generalised epilepsy. As in Takotsubo syndrome in general, the patients were mostly female (84%), with a mean age of 63 years, and the evolution was generally favourable. There was one death and one stroke, and 4 cases were of relapsing Takotsubo after a ...
متن کاملSeizure Associated Takotsubo Syndrome: A Rare Combination
Takotsubo cardiomyopathy (TC) is increasingly recognized in neurocritical care population especially in postmenopausal females. We are presenting a 61-year-old African American female with past medical history of epilepsy, bipolar disorder, and hypertension who presented with multiple episodes of seizures due to noncompliance with antiepileptic medications. She was on telemetry which showed ST ...
متن کاملSeizure-triggered Takotsubo syndrome rarely causes SUDEP
Since almost 20 y it is known that seizures may trigger Takotsubo syndrome (TTS). Since then it has been repeatedly proposed that TTS could be the cause of sudden unexpected death in epilepsy (SUDEP). A review of the so far reported cases of seizure-triggered TTS was carried out to see how often seizure-triggered TTS is fatal. Altogether 59 papers were identified which reported altogether 74 pa...
متن کاملWhat are the similarities between stress, sudden cardiac death in Gallus gallus and sudden unexpected death in people with epilepsy.
Individuals with epilepsy are at higher risk of sudden unexpected death in epilepsy (SUDEP), responsible for 7.5% to 17% of all deaths in epilepsy. Many factors are current associated with SUDEP and possible effect of stress and cardiac arrhythmia are still not clear. Sudden death syndrome (SDS) in chickens is a disease characterized by an acute death of well-nourished and seeming healthy Gallu...
متن کاملSudden unexpected death in epilepsy.
Mortality in people with epilepsy is two- to three-times that of the general population. This can be attributed to epilepsy itself (epilepsy-related death) or to the underlying cause of the epilepsy. Sudden unexpected death in epilepsy (SUDEP) is the commonest cause of epilepsy-related death. It is a syndrome where a person with epilepsy dies suddenly and no other cause of death is found. There...
